What happens in a first session

If you or someone else is in immediate danger, this page is not the right place — here is what to do.

A first session at PHC lasts 50 minutes. You talk, a psychologist listens and asks questions, and together you work out what is going on and what should happen next. Nothing is tested, decided or treated before that conversation.

Before the day: what booking actually involves

Booking at PHC is an enquiry rather than a self-service calendar, and it is worth knowing that in advance so the sequence does not surprise you.

You send us six things: the name of the person who will attend, their age, what is going on in your own words, whether you would prefer a male or female psychologist, whether you want to come in or meet online, and whether the person attending is a minor or elderly. We come back to you by phone or on WhatsApp with a psychologist, a time and a fee. If you agree to all three, you pay a booking payment of half the fee at least 24 hours before the appointment, in cash or by bank transfer, and the slot is held at that point. The rest is paid after the session.

The booking payment is half the fee and is due at least 24 hours before the appointment, because a real appointment is standing empty until it arrives. If it does not arrive the time is not held — you will not have paid anything, so there is nothing to refund, and we will look for another time.

The "what is going on" line is the one people agonise over. It does not need to be well written or complete. "My son is not coping at school" or "I have not felt right since March" is enough to match you to the right person.

Arriving

PHC is appointment-only, at our PECHS clinic and in DHA, Monday to Saturday, 10am to 8pm. Walk-ins are not accommodated at either clinic, so there is no waiting room full of people to sit in.

Bring any reports, prescriptions or school letters that are relevant if you have them. If you do not have any, that is not a problem and nobody will ask why. If you are coming for a child, one parent is usually enough for the first appointment unless we have said otherwise.

If you are meeting online, you will need a quiet place where you will not be interrupted for the hour, headphones if you have them, and a connection you trust.

The 50 minutes

A first session is a conversation, not a procedure. It runs to 50 minutes, and if it goes one to five minutes over you are not charged for that.

Most of it is you talking and a psychologist listening, asking, and occasionally writing something down. They will want to understand what has been happening, roughly how long it has been going on, what it is affecting — sleep, work, study, appetite, relationships — and what you have already tried. They will usually ask about your general health and about any medication you are taking, because both change the picture. They may ask what you are hoping for.

What they are doing while you talk is forming a view of what is actually going on, which is a different job from beginning to treat it. It is why PHC is described as an assessment-first practice and why nothing is administered in that first hour.

Towards the end, the psychologist will tell you what they think and what they suggest: whether further assessment is warranted and which kind, which approach they would recommend, roughly how the work would be structured, or that what you need is somebody else. That last one is a real outcome of a first session, not a failure of it.

What you will not be asked

You will not be asked to complete a questionnaire in a waiting room, score yourself against a checklist, or take a test you have not had explained to you first. If a formal assessment is warranted, it is arranged afterwards and you are told what it measures and why before it happens.

You will not be pushed to talk about something you are not ready to talk about. "I would rather not go into that yet" is a complete answer and a psychologist will move on. A first session is not a test of how much you can disclose, and people who say almost nothing about the worst thing in the first hour are extremely common.

You will not be diagnosed on the spot to make the hour feel conclusive. If a psychologist is not sure yet, they will say so.

What if I cry, or cannot speak, or change my mind?

All three happen, often, and none of them is a problem.

People cry in first sessions and nobody minds. People arrive having rehearsed an explanation and find it will not come out; if that happens, the psychologist will ask smaller questions until it does. People get to the door and want to leave, and if you do want to leave, you can.

If you finish the session and do not want to come back, you do not have to explain yourself. If you want to come back but with somebody else, tell us — you can change psychologist without justifying it at length, and nobody here takes it personally.

What happens afterwards

One of three things, and you will know which before you leave.

Therapy is recommended. You agree a rough frequency and the work begins. Sessions stay at 50 minutes and what happens in them depends on what you came with — CBT, DBT, EMDR, play-based work for a young child, or couples or family sessions if the difficulty sits between people rather than inside one.

Assessment is recommended first. Where a fuller picture is needed — a child's learning difficulty, ADHD or autism, personality, memory and attention — the first session decides which tests are clinically warranted. You will not be given tests you do not need, and you can ask at that point what the rest of the assessment involves.

Somebody else is recommended. If what you need is psychiatric input, a medical opinion or a service PHC does not offer, we say so and point you there.

What PHC does about this

Every route into PHC runs through a first session, and psychologists are matched by speciality and clinical fit rather than by who happens to be free.
